Partnership and getting the stakeholders involved is good, but the novice within the realm of this, can provide off-the-chart-ideas to balance the RCA process. Failure elimination using just the guys who are relentlessly dealing with the problem, are sometimes to deep in the forest to see the trees. The outsider however may just stumble onto the new growth tree in the middle of the underbrush that directs everyone back on the path.... Guenther
Partnerships with our other stakeholders. It's not that we don't know how to solve it, it's getting the cooperation and buy-in to make it happen. That's where most struggle.
Two of the biggest issues I encountered are 1) dealing with frequent stochastic equipment failures, and 2) building a cooperative relationship with suppliers. As a matter of fact, the first issue is the one that drew me into reliability. It's not unusual in complex manufacturing plants to have machines that fail (overcycle, quit, break) dozens and hundreds of times a month. Often, these failures may last for a couple minutes, then mysteriously, things "repair themselves". Traditional RCA doesn't seem to help, either. The second issue is cultural. Dealing with domestic automotive companies, I won't say more . . . . .
