I've been told that the worm had 60 Nm of torque applied for 50 seconds. Hope this helps.

I would Suspect it to be a lubrication issue. Maybe too much friction due to lack of lubrication. 

Many thanks for that Jake. Would the raised edges on the helical gear suggest overloading or a lack of lube? 

Above is a good graphic explaining how the stress is occurring. It could technically be both, however since it is premature wear, it’s hard to really tell. I’ll point you to this article for a great reference, knowledge and understanding.
